Plant Designs, Inc. has always strived to give our clients the most worry-free and comprehensive service possible. If you have any issues with our products or services, we want to know about it.

This form is intended for use by our current clients:  Before you begin, please notify the client contact for your building or department. We designate one person as our contact, usually someone in charge of building maintenance or purchasing. This will also enable us to give information to the proper person in a more efficient manner. 

• Have the contact fill out and submit the Action Form to the right.
  
  
• You may also want to check out our Known Issues list before using this form.
  
  
• If what you want to tell us is really more of a suggestion or a request for enhancement, please use our Suggestion Form instead.
  
  
• When you submit your issue, it will be entered into our database for tracking purposes. The resolution of the problem will be assigned to an appropriate staff member.
  
  
• Periodically we will post notices regarding issues reported by our customers in the Known Issues section.
  
  
• Tips or solutions can be found on the FAQ (Frequently Asked Questions) page.
  
  
• All information given to us is held in the strictest confidence.
